If you are an adopted adult over the age of 19 or an adoptive parent, and the adoption was done through Children's Home Society & Family Services (formerly Children's Home Society of Minnesota) IN MINNESOTA, and you would like to obtain an Non-Identifying Background Registration packet, please email us at pas@chsfs.org with the following information. Please note that we cannot help you if you were placed through a Children's Home Society agency in another state.

- the country of your birth
- your date of birth
- the names of your adoptive parents
- your home phone number (Is it okay for us to identify ourselves and/or leave a message from the agency at this number?)
- a work and/or cell phone number (optional) (Is it okay for us to identify ourselves and/or leave a message from the agency at this number?)
- your gender
- your mailing address
