|  | | Register for International Intermediary Services | | If you are an adoptive parent or an adopted person over the age of 18 and wish to initiate International Intermediary Services, we would be happy to send you a registration packet. Please email your request for information to pas@chsfs.org with the following information.
- the adopted person's full name (present and maiden)
- the country in which the adopted person was born
- the adopted person's date of birth,
- the names of the adoptive parents
- a home phone number (Is it okay for us to identify ourselves and/or leave a message from the agency at this number?)
- a work and/or phone number (optional) (Is it okay for us to identify ourselves and/or leave a message from the agency?)
- the adopted person's gender
- the adopted person's mailing address
- the names and birth dates of any adopted siblings, if known
