Vietnam's Post Placement Requirements

By adopting from Vietnam, parents commit to completing a series of reports on the development of their adopted child; specifically, parents agree to submit one report every six months during the first three years of the adoption, and one report every twelve months thereafter until the child is 18 years old. The Vietnamese law has been changed to read that all yearly post placement reports are due September 1st of each year.

 

In order to fulfill this requirement, CHSFS asks that parents:

            (1) complete the post placement report (available in fillable PDF format)

            (2) print four originals of the completed report

                - the signature of one parent must be notarized on each report.

            (3) print/copy 4 identical photos or photo pages

            (4) Send these items to:

                        CHSFS

Upon receiving the signed reports and photos, CHSFS will be responsible for having the report translated and submitted to the appropriate entities in Vietnam which include the Department of International Adoption (DIA), the People Committee of the province your child was adopted from, and the orphanage your child was adopted from.  CHSFS will also keep one original on file.
